How to provide feedback
We want to know what you think about our services.
At Amity Health we aim to provide the best possible service to our clients. We invite you to make a suggestion, to give a compliment or to make a complaint about our services to help us continuously improve.
How do I provide feedback?
There are two ways you can provide feedback to Amity Health:
- Directly to the appropriate staff member or allied health professional in writing, in person or over the phone; or
- Download and complete a written feedback form. You can either fax or post it to us, or hand it to our Reception Staff in person.
What happens next?
Once we have received your feedback, we will address your comments and if appropriate, take action as soon as possible. If you provide us with your name and an address or telephone number, we will contact you directly to discuss your feedback and inform you of any actions taken.
