AGED CARE

Aged care refers to older people who can no longer live independently or they need help to stay in their homes. People in aged care often face many challenges and our team at Amity Health can help to support these valued members of our community.

For many years we have provided quality aged care services in Albany and the Great Southern.

We partner with a number of residential aged care facilities providing Dietetics, Speech Pathology and in some cases Occupational Therapy services.

We also partner with organisations such as Clarence Estate and Silver Chain to provide these services under home care packages.

  • If you or a family member live in a Residential Aged Care Facility (RACF) in the Great Southern, Midwest, South West, or Wheatbelt region, you may benefit from our mental health program “Connections Count”. Click the button below for our eligibility criteria and further information.

  • At Amity Health, we have a team of care finders in the Great Southern and Wheatbelt regions.

    The care finder program is not for everyone. The program helps vulnerable older people who need support and assistance to access aged care and other services.

    To receive care finder support, a person must;

    1. have limited access to family support and,

    2. be eligible for government-funded aged care.

Under our Fee for Service model, Amity Health can assist you or an older person you know, with their dietary needs, communication and activities of daily living.

CHRONIC CONDITION

So what is a chronic condition? It is an illness that is generally long-term and ongoing and refers to a broad range of conditions such as diabetes, heart disease, mental health and asthma.

If you have been diagnosed with a chronic condition, it's worth getting in touch with us or talking to your GP as you may be eligible for our fully funded Integrated Chronic Disease Care Program.

This program will work with you to reduce risks and improve your overall health, particularly if you have diabetes, cardiovascular disease, respiratory disease, or obesity.

This program is available across the regions of the Great Southern, Coastal, Southern and Eastern Wheatbelt and the South Coastal Goldfields and requires a referral from your GP or other allied health practitioners, and a GP Management Plan.

Within the Great Southern region, we have several fee for service options where our allied health practitioners can provide assessments and support to help navigate your health concerns.

These options include accredited Diabetes Educators, Dietitians, Occupational Therapists, and Speech Pathologists.

DIabetes education

Whether you are at risk of developing Diabetes or are living with Type 1 or Type 2 diabetes, we have a team of Credentialed Diabetes Educators across the Great Southern region that can help you to understand and manage your condition.

We can provide you with support and information on:

  • Type 1 Diabetes - including insulin pump starts, ongoing care continuous glucose monitoring education

  • Type 2 Diabetes - including lifestyle advice as well as medication review and advice in consultation with your GP

  • Gestational Diabetes

  • ICDC (Integrated Chronic Diseases Care)

  • Diabetes Education to groups and individuals on DESMOND (Diabetes WA) & the Smarts Program

In addition, our team of Dietitians can help you with individualised dietary advice for managing Type 1 diabetes, Type 2 diabetes, gestational diabetes and pre-diabetes.

Self-referrals are accepted however if you are living with a chronic condition, we suggest you discuss this with your GP as you may be entitled to claim the Medicare rebate under a chronic disease management plan. Under this plan, if you are eligible, you can receive up to 5 visits within a calendar year.

DISABILITY SERVICES

If you, your child or someone you know has a disability Amity Health may be able to help.

We can provide a range of allied health services to assist a person living with a disability to maximise their potential. We offer predominantly clinic-based appointments for assessment and ongoing treatment, with occasional home/school visits when appropriate. Telehealth is also available as an appointment option.

Whether your needs relate to assistance with communication, movement, sensory abilities, activities of daily life or memory/thinking skills, contact us to discuss how we can help.

Amity Health is an NDIS registered provider with experience in managing autism, hearing and visual impairments, intellectual disability, brain injury and stroke. If you have a mobility impairment that requires you to use a wheelchair, our Albany based clinic now has wheelchair scales.

mental health care

Having good mental health is important at every stage of life, from childhood and adolescence through to adulthood.

If you have concerns about your well-being, we have several options available to you, as outlined below:

  • Please click here to see the counselling options that are available to you:

    This program is NOT suitable for those who are currently in crisis or in need of urgent care.

  • We can provide counselling to address emotional or behavioural challenges, anxiety, depression, stressful life events and any situation that may impact your mental well-being.

    Self-referrals are accepted or see your GP to discuss if a Mental Health Plan may be appropriate for rebates via Medicare or through our funded mental health care program (IPMHC).

    We are fortunate to have Danita Walsh (Art Psychotherapist, Occupational Therapist and Mental Health Clinician) who provides Art Therapy and Chronic Pain counselling as options.

    We are also an approved NDIS provider which means that mental health counselling and art therapy can be accessed using this funding where appropriate.

  • If you or a family member live in a Residential Aged Care Facility (RACF) in the Great Southern, Wheatbelt or Midwest region, you may benefit from our Primary Mental Health Care program “Connections Count”. Please click here for the eligibility criteria.

  • As part of our Fee for Service Mental Health team, Amity Health offers Psychology and counselling services with Registered Psychologist, Brigid Morrigan.

    Delivering psychological and counselling support for clients aged between 10 and 67 years of age, with mental health difficulties including:

    • Anxiety, depression, grief, stress and trauma related difficulties, and other emotional and self-regulation concerns.

OCCUPATIONAL THERAPY SERVICES

So, what is Occupational Therapy? It is a health service that helps people to participate in all the things we do in our everyday lives that help us to live meaningful lives.

Our team of OTs can provide you with a range of services whether you are relatively mobile and active, or reside in an aged care facility.

Have you had surgery for a hip replacement or suffered a stroke or injury? Our OTs can help you to regain or enhance your confidence and skills.

We can also assist you to access adaptive equipment such as mobility aids, to demonstrate its usage and how it can help you.

Self-referrals to see an OT are accepted.

If you are living with a chronic condition, we suggest you discuss this with your GP as you may be entitled to a Medicare rebate under a chronic disease management plan. If eligible, you can receive up to 5 visits within a calendar year.

SPEECH PATHOLOGY SERVICES

Our Speech Pathologist Alanna, can provide a range of services for adults in the community and in aged care facilities with communication and swallowing difficulties including:

  • Assessment and diagnosis of communication and swallow difficulties including rehabilitation services that are tailored to individual needs.

  • Advice on the use of compensatory strategies, including the use of alternative and assistive technology (e.g. communication boards, electronic voice output devices, voice amplifiers).

  • Education to patients, family members and care staff working with the person with communication and/or swallowing difficulties.

Amity Health is a registered NDIS provider.

Self-referrals are accepted however if you are living with a chronic condition, we suggest you discuss this with your GP as you may be entitled to claim the Medicare rebate under a chronic disease management plan and/or a team care plan.
