THE MENTAL HEALTH COUNSELLING PROGRAM

Through our Integrated Primary Mental Health Care program, we offer counselling services for people who are experiencing mental health challenges.

Our clinicians will work alongside you with the aim to help you optimise your mental health and wellbeing.

WHO IS ELIGIBLE?

People who:

  • are struggling with mental health issues

  • are experiencing financial hardship

  • live in the Great Southern or Wheatbelt regions of Western Australia

  • are not accessing other mental health services

  • are aged 7 years and above

OPTION 1: WELLBEING COUNSELLING (self-referral)

This option is available to people 18+ years, and offers 3 sessions that will help you to focus on the here and now, providing you with support, education and skill development to help improve your mental wellbeing.

To access this program please complete the fillable self-referral form below and email to query@amityhealth.com.au

Once your referral has been submitted, we will contact you within 7 business days to discuss how we can help you with your wellbeing plan.

 
 
 

OPTION 2: mental health THERAPY

Sometimes you may need a little more help especially if you’ve previously experienced mental health challenges, or you are currently struggling with your mental health.

Mental Health Therapy is for 7 years of age and above, and includes 6 - 10 sessions. A referral from a GP, Psychiatrist or Paediatrician is required.

OPTION 3: CLINICAL CARE COORDINATION (NORTHAM ONLY AT THIS STAGE)

This service is designed to support GPs in the management of individuals with severe and persistent mental health conditions, for those who are stable, and not in a crisis situation.

We can provide integrated services that cover a range of needs such as:

  • Monitoring of mental and physical health

  • Liaison with other health providers as part of a shared-care arrangement

  • Linking to other services as required, e.g. drug and alcohol, personal support, physical health

  • Provision of health literacy and education to individuals, families and carers

  • Medication management

  • Managing clinical deterioration and facilitating specialist consultation/care
