At Amity Health we provide a range of funded programs for our regional children, adults, families and community.

***Each of these programs requires a referral from a GP or allied health services practitioner and there are certain criteria that your patients must meet to be eligible***

care finder program

At Amity Health, we have a team of care finders in the Great Southern and central Wheatbelt region.

The care finder program is not for everyone. The program helps vulnerable older people who need intensive support and assistance to access aged care and other services.

To recieve care finder support, a person must;

  • not have access to a family member, carer or support person who is able assist, or someone they feel comfortable and trust, and

  • be eligible for government-funded aged care.

Integrated Chronic Disease Care Program

This program is available across the regions of the Great Southern, Coastal, Southern and Eastern Wheatbelt and the South East Coastal Goldfields. Beneficial to your patients who have been diagnosed with diabetes, cardiovascular disease, asthma and chronic obstructive pulmonary disease, or morbid obesity.

Integrated Primary Mental Health Care (IPMHC) Program

This program is available for those in the Great Southern and Wheatbelt regions who are living with mild to moderate mental health issues.

IPMHC PROGRAM IN RESIDENTIAL AGED CARE FACILITIES

Amity Health provide mental health services through the Integrated Primary Mental Health Care program “Connections Count”, to people living in Residential Aged Care Facilities (RACFs) in the Great Southern and Wheatbelt region.
